An Interview with Berkant Karduman on FHIR: Shaping the Future of Health Data Interoperability

Today, we are thrilled to have Berkant Karduman, our Integration Development Manager, join us to discuss one of the most transformative protocols in healthcare IT – FHIR (Fast Health Interoperability Resources). Karduman will shed light on the importance of FHIR in health data interoperability, its real-world applications and how our FHIR Stack product addresses some of the most complex challenges in integrating health information systems. Let’s dive into our conversation. 

Here's the interview:

1. FHIR is often seen as the backbone of modern health interoperability. Could you explain to our audiences what FHIR is and why it has become such a crucial standard for the healthcare industry?

'FHIR, Fast Health Interoperability Resources, is an HL7 standard protocol specifically designed to seamlessly exchange healthcare information. It is unique because it offers a standardized approach to sharing data across diverse health systems, making creating a common language for electronic health records (EHRs) and health information systems easier. The reason why FHIR has gained so much traction is due to its flexibility and modularity. Unlike its predecessors, it combines the best features of previous standards, such as HL7 v2 and v3, while introducing more advanced functionalities, including web-based resources and RESTful APIs.

In the context of our product, FHIR Stack, we leverage FHIR to facilitate real-time communication across clinical and administrative workflows, ensuring that healthcare providers can access the information they need, when they need it and in a format that is universally understandable. This means FHIR is not just a protocol; it’s an enabler for better patient care and more informed decision-making.'

2. What are some of the key challenges healthcare organizations face when implementing FHIR-based solutions and how does Tiga’s FHIR Stack help overcome these hurdles?

‘Implementing FHIR-based solutions is not without its challenges. One of the primary hurdles is data standardization and integration. While FHIR provides a standardized protocol, healthcare data often comes from disparate sources such as patients, labs, imaging centers and EMR systems, leading to inconsistencies, duplications and ultimately impacting the quality of patient care. Moreover, ensuring patient data deduplication, maintaining terminology consistency, managing patient consent and securely sharing sensitive data present further complications during implementation.

Another challenge is the extensibility of the FHIR standard, which, although offering flexibility, can sometimes result in misinterpretations or variations in implementation.

Finally, a significant issue lies in the secure distribution of collected data to other stakeholders, including patients themselves.

At Tiga Healthcare Technologies, we’ve developed our FHIR Stack to address these specific challenges. Our solution acts as a centralized platform that harmonizes and integrates healthcare data from various sources, strictly adhering to FHIR guidelines. With advanced algorithms to prevent data duplication, synchronization features across systems and real-time analytics capabilities, we enable healthcare organizations to achieve seamless interoperability while maintaining a cohesive and reliable health information ecosystem.’

3. How do you see the future of FHIR evolving, particularly with emerging technologies like AI and advanced data analytics?

‘The future of FHIR is promising, especially with the increasing integration of emerging technologies such as AI and machine learning. FHIR’s modular architecture and extensibility make it ideal for supporting AI-driven healthcare applications, enabling more intelligent health information systems that can predict patient outcomes, optimize administrative workflows and even identify potential health risks before they become critical.

At Tiga Healthcare Technologies, we are continuously advancing our FHIR Stack to incorporate these cutting-edge technologies. For example, we’ve integrated an Enterprise Master Patient Index (eMPI) to ensure accurate patient identification and record matching and we’ve implemented integrated terminology management and terminology translation capabilities to maintain consistency across systems. Additionally, our stack includes a robust Role-Based Access Control (RBAC) mechanism and security protocols that support granular control of patient data sharing and consent management, ensuring that sensitive information is always handled with the utmost care and compliance.

By using these capabilities, FHIR will continue to play a pivotal role in creating a more connected and intelligent healthcare ecosystem, paving the way for innovative solutions and improved patient outcomes.’

4. What are the benefits of FHIR in data analytics and how can it enhance decision-making in healthcare?

‘FHIR’s standardized data format enables healthcare organizations to perform advanced data analytics more effectively. By ensuring that data is structured and normalized, FHIR allows for more accurate analysis and insights. This can lead to improved decision-making, where healthcare providers can easily access patient information, understand care patterns and identify areas for operational optimization. In our FHIR Stack, we use these standards to offer real-time analytics that support evidence-based decision-making, facilitating a proactive approach to patient care and population health management.’

5. Can you elaborate on the common use cases of FHIR and how it addresses specific healthcare needs?

‘FHIR is commonly used in scenarios such as patient data exchange between hospitals, integration of lab results with EMR systems and ensuring seamless communication between various healthcare applications. At Tiga, our FHIR Stack addresses specific needs like patient deduplication through eMPI integration, terminology consistency with an integrated terminology management system and secure data sharing using robust RBAC mechanisms. These solutions enable healthcare organizations to build interoperable systems that enhance patient safety and continuity of care.’

6. What are some of the security challenges in sharing health data and how does Tiga’s FHIR Stack ensure data privacy and compliance?

‘One of the major security challenges in health data sharing is ensuring that sensitive patient information is protected and shared only with authorized individuals. Compliance with regulations like GDPR, HIPAA and KVKK requires robust security mechanisms and granular access controls. Our FHIR Stack includes a comprehensive security framework with role-based access control (RBAC), data encryption and patient consent management capabilities. It allows healthcare organizations to confidently share data while maintaining compliance and ensuring that patient privacy is always prioritized.’

7. What advantages does FHIR provide from a patient’s perspective?

‘From a patient’s perspective, FHIR offers increased transparency and control over their own health data. With FHIR-based solutions, patients can access their complete health records, including lab results, medications and care plans, all in one place. It empowers them to make informed decisions about their own health and facilitates better communication with healthcare providers. Our FHIR Stack further supports this patient-centric approach by providing secure patient portals and consent management tools, allowing individuals to decide who can access their health information and how it’s shared.’

In conclusion, FHIR has undoubtedly transformed how we approach health data interoperability. Solutions like our FHIR Stack have the potential to redefine how healthcare providers access, share and utilize health data.
