What is Health Information Exchange?
Health Information Exchange is the secure electronic sharing of patients' health information between different healthcare providers and systems. Through HIE, a patient's medical data can travel from an information system to another system, instead of being trapped inside one healthcare facility's system. In other words, physicians, nurses, and pharmacists can electronically access and share patients' vital information like current medications and laboratory results with HIE. There are mainly three types of HIE:
- Directed exchange: This HIE type, which is also known as push exchange, is the point-to-point transmission of patient data from one provider to another. The sending organization begins the directed exchange. This type is generally used for referrals, discharge summaries, care transitions, laboratory results, and specialist consultations.
- Query-based exchange: When a healthcare provider searches for and retrieves information about a patient from another healthcare organization, it is called query-based exchange. This is particularly useful in emergency care, unplanned hospital visits, and clinical decision support. This HIE type is also known as pull exchange.
- Consumer-mediated exchange: This type makes patients active participants in their health journey; patients control and participate in the exchange of their own health information. Through consumer-mediated exchange, patients can share their data with authorized healthcare professionals or relatives by using a patient portal like our Personal Health Record (PHR) solution.

How Does HIE Work?
The HIE working mechanism consists of different steps. The first step is patient data creation and storing in healthcare systems such as hospitals, clinics, laboratories, and pharmacies. After that, HIE connects these different systems and makes relevant information available for exchange.
The third step involves identifying and matching patients across different healthcare organizations. This step focuses on the patient's demographic information like name, gender, date of birth, address, and so on. Authorization and consent verification, the fourth step, means access checking based on patient consent, organizational policies, and applicable regulations.
At the fifth step, the requested health information is securely transferred from the source organization to the authorized recipient. After this step, the receiving provider uses the data for care delivery, coordination, referrals, or other authorized purposes.
